Development and Pilot of a Telehealth Home-Based Transition Intervention for Pediatric and Young Adults with Cancer

Overview

Primary Objective:

  • To pilot and refine a 3-week hospice telehealth transition program of coordinated hospital/hospice/family visits for children and adolescents/ young adults (AYAs) with cancer and evaluate whether the program (1) is feasible and acceptable for families and providers, (2) achieves high family satisfaction with hospice, (3) fosters hospice and hospital provider interprofessional collaboration, and (4) decreases hospice disenrollment and hospital readmission rates.

Exploratory Objectives:

Feasibility of implementing a telehealth transition program for hospice patients.
