CHOA2460-13:Promoting Transition Readiness among Adolescent and Young Adult Survivors of Childhood Cancer Using SurvivorLinkCSL

Overview

Primary Objective:

  • To assist AYA patients in their use of SurvivorLink to electronically share their treatment history, risk profile, and surveillance plan with primary care providers (PCPs) and future adult LTFU providers

  • To assess impact of the cognitive-behavioral variables (e.g., current transition readiness and knowledge; self-efficacy in managing their healthcare; opinions about need for ongoing LTFU; and neurocognitive late effects) on the ability and willingness of AYA survivors¿ to utilize SurvivorLink

  • To assess impact of the cognitive-behavioral variables (e.g., current transition readiness and knowledge; self-efficacy in managing their healthcare; opinions about need for ongoing LTFU; and neurocognitive late effects) on the ability and willingness of AYA survivors to utilize SurvivorLink
