Overview
Primary Objective:

To assist AYA patients in their use of SurvivorLink to electronically share their treatment history, risk profile, and surveillance plan with primary care providers (PCPs) and future adult LTFU providers

To assess impact of the cognitive-behavioral variables (e.g., current transition readiness and knowledge; self-efficacy in managing their healthcare; opinions about need for ongoing LTFU; and neurocognitive late effects) on the ability and willingness of AYA survivors¿ to utilize SurvivorLink
